🩺 What is Read Coding?

Read Codes are a coded thesaurus of clinical terms used in primary care across the UK. They allow practices to capture key data about patient conditions, symptoms, referrals and treatments in a structured and consistent way.

Although many systems now use SNOMED CT, V2 Read Codes remain an essential part of the data landscape in general practice, particularly for legacy coding, reporting, audits and population health management.

For frontline staff, understanding how to apply Read Codes correctly is crucial for:

• Ensuring accurate patient records

• Supporting clinical safety

• Enabling effective QOF reporting and claims

• Improving referrals, searches, and health data analysis
