North Belfast General Practice Pharmacists recently undertook and completed a project on Type 2 Diabetes in conjunction with Screen Clinical across our federation area. The results, shown on this poster, are great to see across North Belfast participating practices. This project allowed many diabetic patients within the Federation to access improved patient care through review and medication, with close to 2000 medical interventions. Participating practices have been provided with their individual results and are now able to add this to their QI/audit evidence.  Our goal is to now submit this poster for recognition awards for the amazing work undertaken by our practice pharmacists.
